The Ashfield Exercise Referral scheme is a 12 week programme aimed at encouraging referred patients in the Ashfield area to lead a healthy lifestyle through physical activity and exercise.

To join the scheme, simply arrange an appointment with your GP or Practice Nurse to discuss your suitability. If you are suitable, a referral form will be completed on your behalf and sent to Ashfield District Council. You will be then contacted and welcomed to the scheme with further details of the physical activity session most suitable for you depending on your reason for referral and health condition.
